You need to find out the reason that the plaster is failing. It could be caused by damp.
Have a look at the condition of the sand/cement render underneath. If you tap it and get a hollow sound it means that it has blown and needs replacing. If it sounds OK, you might get away with just re-plastering the areas affected.
