Ripped down the ceiling of the front part of the house to uncover some plumbing nightmares not sure if it is that clear from pics but pretty much every joist is no good.Too make maters worse at the front of house they used balloon framing so joists attached directly to 2x4's.I am about to rip out all the plumbing and reroute,my question is should I replace each joist or sister new joists,current joists are 2x8 24 inch on center,8 foot ceilings,thanks :(

It's better to scab on than to remove the joist. Slide a new joist up to the existing joist. Instead of using 4" PVC, you could use 3" PVC.
 
Nightmare for sure, what a hack job. You got the right approach but, keep all holes at least 2" from the bottom of the joists.
