Graham: I have to dissagree with what you call typical. This is a problem for few reasons 1. Attaching level with the deck will require more railing and fence at the level of the deck. This can add to the expence when dealing with glass and aluminum with no real benefit. 2. As the stairs often need to be replaced while the deck itself still has life, removing some of the deck to replace the stairs can be a problem especially if the deck has a membrane on it.
Our standard steps are started down one step, treads are cut 10" deep and we use two 2x6s, 11" gives you the 1" nose. We also use 2x10s for stringers but we also add a 2x12 to the outside of the stingers just cut to fit top and bottom. . This not only adds strength but also gives the railing guys more to bolt to.

We find often home owners often do call the pros for the stairs but to save money they do there own prep work like poring a slab for the stairs to sit on and most time they screw the elevation up. A good explanation of the math would be most helpful.
