I filled this crack with Great Stuff foam to prevent rainwater erosion under the slab (photo attached). Is there a toolable concrete material that can be used as a topcoat to blend this in with the concrete in the carport?

Wrong materal for that job, in fact it can make things worst.
Should have been foam packer rod then the suggested caulking.
That spray foam is fill of tiny voids that will now hold water in like a sponge.
 
Time to yank it all out and start over--backer rod, adhesive caulk or maybe even a 2-part silicone.
