I bought this $50 switch with the expectation it would last a few years. Within the first month, the blue LED stopped working. Now 9 months in, the manual switch no longer works.

Has anyone else see premature failures?

Any know a way to get this covered under a warranty?
 
If it is still under warranty, then take it back to where you got it and have it exchanged under warranty.
Make sure you turn off power before wiring it in, this will mess up the unit.

I'd think a wiring error would have instantly smoked this so maybe you have a mild overload, maybe it's something wrong with the device you are controlling.
If not, early failures suggest a random defect put in at the factory rather than a design problem.
