Is this toilet sweat?

The attached photos show a water stain on a ceiling and the pealing paint that is directly over that stain and on the wall behind the toilet.

Is the wall deteriorated by the toilet sweating? Or is there another problem? The valve itself doesn't seem to be leaking. This has been an ongoing issue,I can see that the previous homeowner patched the ceiling at the same location.

How would you approach this? Open the ceiling or open the wall or both? I also plan to insulate the tank regardless.

I use oil based paint in all bathrooms, so the prep may well be the culprit.

The ceiling may be from a leaking wax ring.
 
It could be spray inside the toilet tank . open and watch it fill , the water should not spray up in any manner.
 
I would agree with billshack where its affecting above your toilet supply if that's not you would have to open the wall
 
Another thing to look at is your roof boot. I have had a few of the rubbers crack at the boot and water follows the waste line vent pipe down.
