Trying to identify kitchen faucet

I have a faucet in my kitchen sink that has slowly over time leaking and needs a replacement part. I'm not the original owner so didn't purchase the faucet and after inspection cannot locate any manufacturer labels. I believe this faucet was installed in 2002 as that is the year the house was built. I've included photos of the faucet as well as the connection underneath the sink. Any help is greatly appreciated!

Firstly, I thought it's a faucet for filtered water, but I think, it's better to remove it and change.
You may try to find a new useful kitchen faucet somewhere here or on any other sources like this. I've bought a new kitchen faucet recently with a retractable hose and it's one of the most comfortable things in my kitchen now.
 
It is a dedicated faucet, for either instant hot water, reverse osmosis filter, or a water filter with a chiller.

You can buy new faucets like that anywhere.
 
Home Depot Carrie's them in all different styles, colors and connections.
