What to do with water damaged walls?

In my master bedroom the past owners painted over water damage. What is the best way to clean this up? I have the same paint on hand. The image is attached to the link below. Thanks for any advice!

Dumb question but - do you know for sure that it's water damage? No signs of swelling or crumbling in the wall, and i would think with that much water, the ceiling would be ready to crumble.

how long has it been like that?
 
It looks like condensation running down. Is that section of wall colder than on either side or right below? Is that an exterior wall?
It looks as if someone may have painted over wallpaper or paste residue and that wet look is due to condensation being absorbed by paste under the paint. If you take your fingernail to one of those lumps, it the lump soft?

What's your location?
Looks to me like ice dams on the roof causing water to back up under the shingles.
Been in the attic and checked for water damage?
