Default Paint peeling

I will be repainting a large room and the woodwork has peeling paint issues. How would you approach this? Sand it down or repair and paint over?

I've attached pictures to help with a solution.

Also, it's an older house and a hallway I'm repainting the woodwork. I feel it has too many layers of paint on it. If I add another layer I feel it's too much paint. How would you go about removing layers of old paint? Sand? Strip? Paint over again?

Thanks! I'm a painting newb trying to get it right.

Welcome. First question we have to ask: how old is the house? Could be that the old paint contains lead, and that would affect the method needed to remove it.

If that pic is on the interior, you have some serious issues
