Chain keeps coming off the chainsaw

Bent bar, pinched gears, bad bearings or roller guides.

OP here with an update.

The bar is fine, but had some sawdust in the groove.
The chain was missing a few rakes too.
Cleaned the bar, and switched out the chain and ran good.
