Nothing short on removing and replacing will eliminate the puddles.

Removing the driveway? How about mud jacking? I know I can't replace the steps myself ...estimate cost to have someone do it?

No, not the entire driveway.

You have obvious areas of subsidence, which were probably cause by improper compaction. It's now compacted, so, square cut it and either re-pour it or add a few decorative spreaders between the sections, which will also reduce the area of future adverse effects.

The steps appear to be intact and the sidewalk is the element that has moved, so it's a section of sidewalk which needs to be replaced.

Costs are hard to predict here. Every area has it's own economy and we don't even know which city you're in. The best we can do (and Snooby has already started) is give you some ideas on how it may be fixed. Then, when you talk to a pro, you'll have some background info to help you decide.
